Massachusetts. Municipal Clerks. Vital statistics--notices of intention of marriage (R-105) (town clerk scheduled item).

Each town clerk shall receive or obtain and record facts relative to marriages in his town (MGLA c 46, s 1). Notice of intention of marriage (currently Registry of Vital Records and Statistics form R-105, per s 16) is completed and filed with clerk at least three days before the clerk may issue the certificate of marriage or marriage license ((M-Ar)RS2.126) (MGLA c 207, ss 19-20). Information includes date of record; for groom and bride the following: name, surname after marriage, date of birth, occupation, residence, number of marriage and marital status, birthplace, names of parents, and sworn signature, attested to by clerk; also issue date of marriage certificate.
